What Are the Responsibilities of an NDIS Support Coordinator?


A NDIS Support Coordinator takes on many roles and responsibilities when it comes to helping their clients navigate the NDIS system and connecting them with relevant support services. The key responsibilities include:


Initial Assessment and Goal Setting


  • YOU Queensland conducts an initial assessment with the client to understand their circumstances, goals, strengths, and challenges.
  • Support Coordinators work collaboratively with clients to establish clear, measurable goals and develop a support plan tailored to their needs and aspirations.


Information and Referral


  • YOU Queensland provides information and guidance to the client about available support services, programs, and resources within the community.
  • Support Coordinators assist the client in accessing appropriate services and making informed decisions about their support options.


Service Coordination and Planning


  • Support Coordinators work closely with the client, their families, caregivers, and service providers to coordinate and integrate support services in accordance with the client's support plan. Examples of support services include those that help the individual live more independently and enhance their social and community participation.
  • They assist the client in navigating the complex service system, accessing funding streams, and arranging appointments, assessments, and referrals as needed.


Advocacy and Empowerment


  • YOU Queensland advocates for the client's rights, preferences, and choices throughout the Support Coordination process.
  • Support Coordinators empower clients to actively participate in decision-making, assert their needs, and advocate for their interests within the service system.


Monitoring and Review


  • YOU Queensland regularly monitors the client's progress towards their goals and reviews their support plan to ensure it remains relevant and effective.
  • Support Coordinators collaborate with the client to identify any changes in needs or circumstances and make adjustments to the support plan as necessary.


Capacity Building and Skill Development

YOU Queensland supports clients in developing the skills, knowledge, and confidence to self-manage their support needs and navigate the service system independently.


Support Coordinators provide coaching, education, and resources to empower clients to build resilience, self-advocacy, and problem-solving skills.


Collaboration and Partnership


  • YOU Queensland collaborates with a network of service providers, community organisations, government agencies, and other stakeholders to ensure holistic support for their clients.

  • Support Coordinators foster partnerships and networks to enhance the availability,  accessibility, and quality of support services within the community.

You are eligible for Support Coordination services if your NDIS plan specifically includes Support Coordination funding. We recommend checking your plan or contacting your NDIS planner to confirm your eligibility.


there are three levels of Support Coordination with the NDIS, which are:


  • Level 1—Support Connection: This program helps participants understand and use their NDIS plan and empowers them to make informed decisions about the support services they require.
  • Level 2—Coordination of Supports: A higher level of assistance, the support coordinator works closely with participants to directly connect them with the support services they require. Ongoing support is provided to ensure the seamless implementation and management of their NDIS plan.
  • Level 3—Specialist Support Coordination: This is the most involved level of support coordination, usually for individuals with complex needs. A Specialist Support Coordinator with the NDIS has focused expertise on specific areas of need, such as mental health or individuals with severe disabilities and connects them with complex support services that helps them achieve their goals.


As Support Coordinators, we're able to connect participants with a range of support services based on the specific funding in their NDIS plan. These support services may include (but are not limited to):


  • Personal Care Services: Assistance with daily activities like showering, dressing, and personal hygiene.
  • Allied Health Services: These include physiotherapy, occupational therapy, speech therapy, and psychology.
  • Community & Social Participation: Programs that promote engagement in social or community activities.
  • Household Assistance: Help with cleaning, meal preparation, and other household tasks.
  • Transport Support: Support getting to and from community activities, work, or appointments.
  • Home Modifications: Adjustments to your home to improve accessibility and safety.
  • Assistive Technology: Access to equipment and technology that aids independence.


We offer our NDIS support services all across the Wide Bay region, including the following areas:

  • Murgon
  • Cherbourg


It's easy to get the two roles mixed up, although they have entirely different responsibilities under the NDIS umbrella. A support coordinator helps the participant understand and navigate their plan and, depending on the level of Support Coordination outlined in their NDIS plan, will also connect participants with support services. Our Support Coordinators will also assist you to connect with services that have been funded in your plan. We will assist you to stay within your budget allowing your plan to work as it should. Our Support coordinators will report on your progress to the NDIS and assist you to make changes to your plan as the need arises. 


A Plan Manager, on the other hand, is focused on managing the financial aspect of your NDIS plan, such as paying invoices and keeping track of your funding. Both roles are crucial, but they serve different purposes to help you get the most out of your NDIS plan.


Support Coordination services typically last for the duration of the participant's NDIS plan period. However, the exact length depends on the individual circumstances of each participant and the specific amount of funding allocated for Support Coordination within their NDIS plan. The level of Support Coordination can be reviewed and adjusted during plan review meetings to ensure it is well-aligned with their current needs.
